Comments:
An unannounced monitoring inspection was conducted on April 12, 2019, between the hours of approximately 10:30 a.m. and 4:30 p.m. Sixty-two (62) children were enrolled and forty-eight (48) children, in six classrooms, were in the care of nine staff members during the on site inspection. Three additional staff members were present and assisted, as needed. Six children's records and six staff records were reviewed and/or updated. Background clearances were reviewed in an additional six staff records. Two children's medications and accompanying authorization forms were reviewed. Safety equipment (first aid kits, flashlights and radio), posted information (license, inspection documentation, menu, daily schedules, emergency evacuation diagrams/plan and emergency telephone numbers) and facility documents (injury reports, insurance verification, infant daily records and emergency drills - evacuation, shelter-in-place and lockdown) were observed/reviewed. The daily schedules accurately reflected the activities provided on this date. Lunches were provided by the children's parents. Staff/child interactions were observed to be positive and supportive of children's individual needs. The facility was observed to be clean and supplied with a variety of age/stage appropriate learning and play materials/toys. Program observation was completed. Activities observed on this date included: indoor play (toys and table activities); outdoor play; stories; restroom and handwashing procedures; lunch; infant care, naptime and pick-up/departure of children. The program continues to maintain substantial compliance with the licensing standards; two violations were cited during this inspection. Standard #: 22VAC40-185-160-A
Description: Based on review/update of five staff records and discussion with staff on April 12, 2019, the program failed to ensure documentation of a negative tuberculosis (TB) screening was submitted by one staff member no later than 21 days after employment.

Plan of Correction: The TB screening will be located or a new screening will be obtained by the employee. In the future, TB screenings will be submitted in a timely manner.

Standard #: 22VAC40-191-60-C-2
Description: Based on review/update of five staff records and discussion with staff on April 12, 2019, the program failed to obtain central registry findings for two staff members within 30 days of employment. Evidence: 1. Update of staff #2's record, on this date, determined that the central registry finding was dated more than 30 days after employment (hire date - 12/05/2017 and date of central registry finding - 4/10/2019). 2. Review of staff #5's record, on this date, determined that the central registry finding was dated more than 30 days after employment (hire date - 11/26/2018 and date of central registry finding - 1/03/2019).

Plan of Correction: Staff #2's central registry clearance could not be located and a new request form was submitted and a finding obtained. Staff #5 completed the central registry request form incorrectly, it was returned and resubmitted for processing. In the future, continued effort will be made to obtain central registry findings in a timely manner.
