Comments:
Today an unannounced Monitoring Inspection took place between the hours of 9:50am and 3:30pm. At the start of the inspection there were a total of 85 children with 13 staff; all ratios were incompliance. A total of 5 children?s files and 5 staff files were reviewed. The infants were observed receiving bottles, diaper changes and playing on the floor. The toddlers, preschoolers and school aged children were observed practicing their colors, singing songs, participating in a STEM class, eating lunch and other organized activities. 12 medication and authorizations were reviewed. Positive interactions between children and staff were observed. This week is spring break; school aged children were observed in the all-day program. Violations:
Standard #: 22VAC40-185-240-A
Description: Based on record review and interview, staff have not received the required training by the end of their first day assuming job responsibilities. Evidence: Staff #1 (date of hire: 4/5/2019) and Staff #2 (date of hire: 3/25/2019) have not completed the recognizing child abuse and neglect and the legal requirements for reporting suspected child abuse as required by 63.2-1509 of the Code of Virginia.

Plan of Correction: I'll have these staff complete the training within the next two weeks. In the future, I'll provide the online link to all new staff upon a job offer.

Standard #: 22VAC40-185-500-A
Description: Based on observation, hand washing procedures were not followed. Evidence: 1. 3 out of 3 children did not have their hands washed after their diaper changes in the Infant 3 classroom. 2. Staff #6 did not wash their hands after completing 3 out of 3 diaper changes.

Plan of Correction: This staff member was spoken to during the inspection.

Standard #: 22VAC40-185-500-B
Description: Based on observation, diapers were not disposed of in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: Three children were observed having their diapers changed in the infant c classroom. At the end of each diaper change, Staff #6 used her hand to open the trash can to throw out the dirty diaper.

Plan of Correction: This staff member was spoken to during the inspection.

Standard #: 22VAC40-185-510-C
Description: Based on observation, the center did not return expired medication to the parents. Evidence: Child #10's emergency medication expired 2/28/2018 and was observed in the child's classroom.

Plan of Correction: I will return the expired medication to the parents today.

Standard #: 22VAC40-185-510-J
Description: Based on observation, medication was not kept in a locked place using a safe locking method that prevents access by children, Evidence: Child #2, Child #6, Child #7 and Child #9's emergency medication is kept in a zip lock bag within a backpack in the classroom. The backpacks are not kept in a locked location. Child #8's emergency medication is kept in a zip lock bag within a locked cabinet in the classroom. During today's inspection, the key was left in the lock of the cabinet making the medication accessible.

Plan of Correction: We will order additional secure lock bags and put all emergency medications in the secure bags.
