Comments:
A Subsidy Health and Safety monitoring inspection was conducted on 4/17/2019 to review supplemental health and safety requirements. This inspection began at 9:50am and ended at 3:30pm. There were 13 staff with 85 children in 9 classrooms at the time of the visit. At the start of the inspection, infants were observed receiving bottles, diaper changes and playing on the floor. Toddlers, and preschoolers were observed learning their colors from flashcards, singing songs and eating lunch. This week is spring break; school age children were present and playing independently in the school aged classroom. A STEM class was taking place and the children were learning about wind blowing over houses made out of straw vs. blocks. Children's files, emergency preparedness procedures, fire drill log were reviewed. The physical plant and playground were inspected. There were 12 medications on site at this time. A licensing inspection was also conducted on this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Thank you for your cooperation today. Violations:
Standard #: 22VAC40-665-540-A
Description: Based on record review, staff has submitted documentation of a tuberculosis (TB) screening that was completed more than 30 days prior to coming into contact with children. Evidence: Staff #1 (date of hire: 4/5/2019) TB screening is dated 12/18/2018.

Plan of Correction: I wasn't aware that there was an update to the subsidy standards. We'll have this staff member get an updated screening.

Standard #: 22VAC40-665-770-A
Description: Based on observation and interview, the vendor cannot implement their written procedure for the continuity of operations to ensure that essential functions are maintained during an emergency. Evidence: The vendors emergency plan to ensure continuity of essential functions has a checklist of all emergency supplies and states that all supplies will be located in a central location to be easily accessible during an emergency. The emergency supplies is not currently located in a central location.

Plan of Correction: I will purchase all of the emergency supplies as well as a storage container to keep all supplies centrally located.
