63.2 Facilities & Programs.
The inspector conducted an unannounced mandated inspection from 10:00 a.m. until 1:00 p.m. There were 17 children in care with staff supervising at all times. Children were observed a circle time, washing hands and going to lunch, and playing outside. No medications are being administered at this time. Five child records were reviewed. The violations are identified on the violation notice. Please call your inspector at 276-698-9981 with any questions.
Standard #: 22VAC40-185-40-G Description: Based on record review, the center did not have proof of child identity for a child. Evidence: 1. The inspector reviewed five records. One of five records (#5) did not have documentation of proof of birth. Plan of Correction: Staff will ask parents for proof of birth.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center failed to maintain the center in a safe condition. Evidence: 1. The inspector observed a loose wheel with the bolt missing on the red trykes on the playground. Plan of Correction: Maintenance will be notified and the wheel fixed.
Standard #: 22VAC40-185-330-B Description: Based on measurement, the center failed to maintain resilient surfacing depth requirements. Evidence: 1. The inspector measured mulch around the slide and the climbing equipment and it measured 2-3 inches. Plan of Correction: Staff said the main office was aware and that mulch had been ordered.
