Comments:
An unannounced monitoring inspection was conducted with the licensing consultant and center director on April 16, 2019. There were 97 children, ranging in age from 3 months to five years, in care with 15 staff actively supervising. The children were learning about bees, playing outdoors, and eating lunch. Lunch served today: pasta with meatballs, pineapple and mango, and milk. The center is nicely decorated with the children's artwork throughout. Eighteen medications for 11 children were reviewed - please see Supplemental Summary for details. Four new staff files and ten children's files were reviewed and are in compliance.

Standard #: 22VAC40-185-510-C
Description: Based on review of medications, the facility failed to return outdated medication to the parent. Evidence: the medication for Child F expired 1/2019 and one medication for Child H expired 2/2019 and two other medications for Child H expired 3/2019. The medications were not returned to the parent or disposed of by the center.

Plan of Correction: Center director will return the medications to the parents.

Standard #: 22VAC40-185-510-D
Description: Based on observation and staff review, the facility failed to have documentation of a medication authorization for each medication on site. Evidence: the Benadryl cream for Child A did not have written permission from the parent for the facility to administer.

Plan of Correction: The medication will be sent home or disposed of as it is not being used.

Standard #: 22VAC40-185-510-N
Description: Based on review of medications, the facility failed to notify the parents that the medications needed to be picked up within 14 days of the when the authorization for a medication expires. Evidence: the short term medication authorization completed by the parent for a non prescription medication for Child B expired 8/26/2018. The long term medication authorizations for Child H's three emergency medications expired 3/7/2019. The long term medication authorization for Child G's emergency medication expired 12/22/2018. The medications were not returned to the parent within 14 days of the expiration of the authorization.

Plan of Correction: The center director will return the medication to the parents or obtain another authorization.
