The following unannounced complaint inspection took place on 04/17/2019, between the hours of 8 am through 9 am, on site at the child day center located in Alexandria, Virginia. Standard #: 22VAC40-191-50-A-1-a-2- Complaint related: Yes Description: Based on review and interview, an unsatisfactory sworn statement or affirmation was accepted by the center. Evidence - 1. During a review of a sworn statement or affirmation signed by Staff #1 and dated 02/18/2019, to the following question #3 Have you ever been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth of Virginia? Staff #1 answered "Yes (in Virginia)." 2. This form was completed on Staff #1's date of hire of 02/18/2019 and accepted by the center. 3. The Center Director stated (by email and during a telephone interview conducted on 04/16/2019) that Staff #1's first day of work was 02/25/2019. Staff #1 stopped working on 04/12/2019. Plan of Correction: Staff #1 is no longer working at the center as of 04/12/2019.
Standard #: 22VAC40-191-50-A-2-a Complaint related: Yes Description: Based on review, a copy of the department's child protective services check form was returned to the local Department of Social Services indicating that, as of the date on the reply, the individual whose name was searched was identified in the Central Registry of Founded Child Abuse/Neglect Investigations as an involved caregiver with a founded disposition of child abuse/neglect. Evidence - 1. During a review conducted on 04/16/2019, in a letter from the Office of Background Investigation - Central Registry Unit, dated 04/11/2019, it stated that a search resulted in a MATCH of Staff #1 who is listed in the Child Abuse/Neglect Central Registry with a founded disposition of child abuse/neglect. 2. This MATCH corresponds with a sworn statement or affirmation signed by Staff #1 and dated 02/18/2019, in which Staff #1 answered "Yes (in Virginia)" to the question - Have you ever been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth of Virginia? Plan of Correction: Staff #1 is no longer working at the center as of 04/12/2019.
