Comments:
A Subsidy Health and Safety inspection was conducted on 4-18-2019 to review supplemental health and safety requirements. Which play groups, art time, games, personal hygiene, lunch time, and an egg hunt were observed. Children were observed; and staff and children files and other documentation were reviewed. Thirty-seven (37) children, aged 6 years-11 years, were found to be in care with 4 staff, and administrative staff. Fourteen (14) of the children went on a field trip. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice.

Description: Based on review of children records and discussions with administrative personnel the vendor failed to maintain compliance with this standard regarding allergy action plans. Evidence: The records for children B and D did not have the allergy action plans as required for medically diagnosed food allergies.

Plan of Correction: These plans will be obtained immediately from the parents/physician and placed in the file for review and compliance. Parents were notified during the inspection of the need for this document.

Description: Based on review of documents and discussions with administrative personnel the vendor failed to maintain compliance with this standard as required relating to yearly lock down drills. Evidence: The vendor did not have documentation of any practiced lock-down drill for the year 2018 as required.

Plan of Correction: A lock-down drill will be conducted for 2019 and documented as required for future compliance.
