Technical Assistance:
Had a discussion with the center director regarding operating a child care program under the terms of the WTS license. Anytime children are being cared for licensing standards apply. Discussed staff awareness of how many children are present at all times.

Comments:
Today an unannounced Monitoring Inspection took place between the hours of 9:20am and 11:30am. At the start of the inspection there were a total of 15 children with 1 staff; all ratios were incompliance. A total of 4 children?s files and 3 staff files were reviewed. There were five medications and authorizations reviewed. The children were observed playing hockey and kickball in the gym, watching a movie and leaving to go on a golfing fieldtrip . Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, each staff member did not submit documentation of a negative tuberculosis (TB) screening within the first 21 days of employment. Evidence: Staff #1 (date of hire: 6/11/2018) file does not contain documentation of a TB screening.

Plan of Correction: I have been requesting this documentation from this staff person. This staff person's last day of work will be next week. If they end up coming back they will not be able to return until this documentation is provided.

Standard #: 22VAC40-185-50-A
Description: Based on observation, children's and staff records were not treated confidentially. Evidence: The Supplemental page (containing children's and staff names) from the most recent inspection is currently posted along with the most recent Summary and Violation Notice. The licensing inspector had clearly documented "Do Not Post" at the top of the Supplemental page.

Plan of Correction: This was corrected during today's inspection.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview, the center did not maintain records with all required information. Evidence: 1. 11 out of 15 children present today do not have records. The center director stated that she has phone numbers for the children's parents on the computer, but thought drop-in children did not need complete records. 2. Child #1 file does not contain documentation of first date of attendance or documentation of viewing proof of the child's identity and age. 3. Child #4 file does not contain documentation of an annual update.

Plan of Correction: I will email the parents today requesting this information.

Standard #: 22VAC40-185-510-A
Description: Based on record review, medication was kept on-site without written parental authorization to administer. Evidence: Child #5 and Child #6 both have emergency medication kept on-site in case of an accidental ingestion of an allergen. There is no written parental authorization to administer each medication.

Plan of Correction: The center director emailed the children's parents during the inspection requesting medication authorization forms.

Standard #: 22VAC40-185-510-C
Description: Based on observation, the center did not follow policies for administering medication. Evidence: Child #5's medication A expired 12/2018 and medication B expired 3/2019. Child #6's medication A expired 8/2018.

Plan of Correction: The center director emailed the children's parents during the inspection requesting new medications.
