Comments:
An unannounced monitoring inspection was conducted on April 15, 2019 from approximately 3:22 pm to 6:05 pm. One staff was present with thirteen children at the onset of inspection. Additional staff and children arrived during inspection. Five children's records were reviewed. Required information to be maintained on site as well as training was reviewed for the three staff present on this date.

Standard #: 22VAC40-185-60-A
Description: Based on review, facility did not ensure 1 of 5 children's records has documentation of required information. Evidence: On April 15, 2019, the record for child #2 did not have documentation of a second emergency contact.

Plan of Correction: The Center Supervisor will ensure participant's emergency contact information is documented properly.

Standard #: 22VAC40-185-260-A
Description: Based on review, facility did not ensure there is an annual fire inspection report from the appropriate fire official having jurisdiction. Evidence: On April 15, 2019, the fire inspection report available for review was dated February 7, 2018 which is more than a year old.

Plan of Correction: The Center Supervisor will ensure the most up-to-date fire inspection is posted.

Standard #: 22VAC40-185-340-C
Description: Based on observation, facility did not ensure there is one staff member and an immediately available staff member, volunteer or other employee present when one or more child are present. Evidence: On April 15, 2019 at 3:22 pm, staff #1 was present with thirteen children. No other staff member, volunteer or other employee was present with direct means for communication between the two of them. Staff #2 arrived at approximately 3:39 pm.

Plan of Correction: Youth Programs maintains appropriate staff to participant ratio as a basic safety standard. In the event of an unforeseen staff absence, a substitute staff is secured and on site as quickly as possible.

Standard #: 22VAC40-185-550-D
Description: Based on review, facility did not ensure a minimum of two shelter-in-place drills are practiced per year for the most likely to occur scenarios. Evidence: On April 15, 2019, there was documentation of one shelter-in-place drill in 2018.

Plan of Correction: The Center Supervisor will ensure all drills are documented after completing.
