Comments:
An unannounced monitoring inspection was conducted today with the director. There were seventy-two children with 12 staff providing supervision during the initial walk through. In the various classrooms the children were eating breakfast ( pineapple, cereal and milk), playing with table top games and participating in circle time. Seven medications for six children were reviewed today as well as 8 staff files and 7 children's files. A sample size of injuries/accidents were also reviewed and the center is up-to-date with their escape drills and shelter in place drills. Arrival time: 7:25 am Departure time: 11:45 am

Standard #: 22VAC40-185-190-A
Description: Based on an interview with the director and a review of the file, it was determined that the director did not have documentation to show that she meets the educational requirements for that position. Evidence: The director could not produce any documentation to show that she has the education for said position.

Plan of Correction: I plan to get a copy of my transcripts, additionally I will be completing my CDA soon so one way or the other I will have the documentation to show I am qualified for this position.

Standard #: 22VAC40-185-500-B
Description: Based on observation, it was determined that the diapering pad in three classrooms were being used as a storage location and not solely for diapering as required. Evidence: In Infant C classroom a child 's hair bow was stored on the diapering pad; in infant A classroom a pillow and two high chair covers were stored on a diapering pad and in the toddler classroom a pair of keys were stored on the diapering pad.

Plan of Correction: I will retrain staff on not storing items on the diaper pad.
