Comments:
The licensing inspector conducted an unannounced monitoring inspection on 04/17/2019 from 9:59am to 1:35pm. The children were observed playing in centers, playing on the playground, participating in show and tell, brushing their teeth, eating lunch, and napping. A formal observation was completed with the preschool and infant/toddler classrooms. Lunch consisted of bologna and cheese sandwiches, tater tots, apple sauce, and milk. The menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today. Five children's records were reviewed. Staff records are kept off site and will be reviewed on a later date. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 03/26/2019 Last shelter-in-place drill: 03/19/2018 Last fire inspection: 2/11/2019 Last health inspection: 11/27/2018 Today, the following child to staff ratios were observed: Rm. #1 (Threes and Fours) - 12:3 Rm. #2 (Threes, Fours and Fives) - 12:3 Toddlers - 8:2 Infants - 3:2 The violations from the previous inspections were checked for corrections. Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation, the center did not ensure hazardous substances such as cleaning materials were kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. Classroom #1 had a cleaner located in an unlocked cabinet within the reach of children in care. 2. Classroom #2 had a cleaner on top of a shelf out of the reach of children.

Plan of Correction: Corrected during the inspection. Site administrator will do a daily inspection and will remind staff of the importance of locking cleaners.

Standard #: 22VAC40-185-290-3
Description: Based on observation, the center did not ensure that electrical outlets had protective covers that are of a size that cannot be swallowed by children. Evidence: Classroom #1 had one uncovered out located on the lower part of the wall within the reach of children and two uncovered outlets located on a surge protector near the computer station.

Plan of Correction: Corrected during the inspection. Site administrator will do a daily inspection and will remind staff of the importance of covering outlets.

Standard #: 22VAC40-185-450-C
Description: Based on interview, the center did not ensure that crib sheets were cleaned and washed daily. Evidence: Staff #4 stated that the infant crib sheets are washed and changed weekly unless necessary. Crib sheets should be cleaned and washed daily.

Plan of Correction: We will ensure that there are enough clean sheets to be changed daily instead of weekly.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not ensure that disposable diapers were disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The inspector observed several soiled diapers in a trash can located in the toddler restroom. The trash can was not leak proof or foot operated.

Plan of Correction: I will ensure that the staff have the proper diaper disposal system and encourage the staff to us it.
