The licensing inspector conducted an unannounced monitoring inspection on 4/22/2019 from 10:58am to 2:45pm. The children were observed playing on the playground, eating lunch and napping. A formal observation was completed with the infant and four year old classrooms. Lunch was provided by each child's parent. The morning and afternoon snack menu was posted. Interviews were held with staff throughout the inspection, and the inspector interacted with children in each classroom when appropriate. All classrooms and playgrounds were inspected today as well as the center's vans and mini bus. Five children?s records and five employee records were reviewed during this inspection. The center administers medication; however they did not have any being given at this time. Ten injury reports were reviewed as well as the center?s injury prevention plan which was updated 03/2019. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 03/29/2019 Last shelter-in-place drill: 09/17/2018 Last fire inspection: 09/10/2018 Last health inspection: 7/11/2018 Today, the following child to staff ratios were observed: Infants - 9:3 Toddlers ? 7:4 Twos - 12:2 Threes - 10:2 Fours ? 15:2 The violations from the previous inspections were checked for corrections. Standard #: 22VAC40-185-160-A Description: Based on review, the center did not ensure that each staff member submitted documentation of a negative tuberculosis screening within 12 months prior to or 21 days after employing or volunteering. Evidence: 1. The record of staff #1 (start date: 4/1/19) had documentation of a negative tuberculosis screening dated 12/2/17. 2. The record of staff #3 (start date: 1/29/19) had documentation of a negative tuberculosis screening dated 11/2/17. TB screenings should be completed no more that 12 months prior to employment. Plan of Correction: Two staff will be getting their TB test within this week.
Standard #: 22VAC40-185-270-A Description: Based on observation, the center did not ensure that areas and equipment of the center were maintained in a clean, safe and operable condition. Evidence: 1. The center's playground equipment and fence had areas with sharp splintering wood and protruding nail heads. 2. The two year old class room had chipped and peeling paint. Plan of Correction: Maintenance will fix those areas as soon as possible.
Standard #: 22VAC40-185-440-J Description: Based on observation, the center did not ensure that occupied cribs have twelve inches of space between the sides and ends of cribs except where they touch the wall. Evidence: The inspector observed 2 occupied cribs. Both cribs did not have 12 inches of space between the sides. Plan of Correction: Corrected during the inspection.
Standard #: 22VAC40-185-500-B Description: Based on observation, the center did not ensure that disposable diapers were disposed in a leakproof or plastic-lined storage system that is either foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Evidence: The toddler classroom had a disposable diapers located in a disposal system that was neither foot operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. Plan of Correction: Corrected during the inspection.
