Comments:
The licensing inspector conducted an unannounced renewal inspection on Monday, April 22, 2019 from approximately 10:20 AM until 1:10 PM. The following ratios were observed with a total of 19 children present during the inspection: Infants and Toddlers- 4 children with 1 staff Twos/Threes? 7 children with 1 staff School Age - 8 children with 1 staff During the inspection, the children were observed playing with toys, resting in cribs, singing songs, looking at books, coloring, playing on tablets, washing their hands and taking bathroom breaks, eating lunch and resting at naptime. Areas of the center including classrooms, hallways, bathrooms and outdoor play areas were inspected. The center was equipped with toys and supplies, and items were available to the children. Transportation is provided and the vehicle and transportation documents were reviewed. Management stated medication is not administered at this time. During the inspection, five children's and five staff records were reviewed. The following information was reviewed: emergency supplies, emergency drills, daily attendance, infant daily sheets, parent agreements, liability insurance policy, required postings and staff training information. The risk profile tool will be used for the licensing recommendation by the licensing department. Violations:
Standard #: 22VAC40-185-140-A
Description: Based on review of five children's records and interview, the center did not obtain documentation of a physical exam for each child within the required time frame.

Evidence:
During the inspection, the licensing inspector observed the records for Child #1 (start date 3/11/19), Child #2 (Start date 3/6/19), Child #4 (start date 3/18/19) and Child #5 (start date 3/1/19) did not contain documentation of a physical exam. Staff #1 confirmed this information during the inspection. Each child is required to have documentation of a physical exam prior to or within the first 30 days of attending the center.

Plan of Correction: We will have the parents submit copies of physical exams within one week.

Standard #: 22VAC40-185-550-H
Description: Based on interview, the center did not ensure the required documents were kept in the vehicle used to transport children.

Evidence:
During the inspection, the licensing inspector asked Staff #1 if the routes to schools and other frequently visited places were kept in the vehicle. Staff #1 stated the schools are located nearby and the routes were not kept in the van. Potential shelters, hospitals, evacuation routes, etc., that pertain to each site frequently visited or of routes frequently driven by center staff for center business (such as field trips, pickup/drop off of children to or from schools, etc.) is required to be documented and must be kept in vehicles that centers use to transport children.
