63.2(17) License & Registration Procedures

Comments:
An unannounced monitoring inspection was conducted today, April 19, 2019 to determine the center's compliance with licensing standards. The inspection was initiated at 10:00 am and concluded at 12:45 pm. Upon the inspector's arrival, children ages 2 years through 5 years were engaged in circle time. Children sang songs, talked about Easter and read the story" It's not easy being a bunny". Later children painted with watercolors. The infant and toddlers played with toys and were observed during lunch time. Diapering was also observed with hand washing being conducted. The two through five year old children had time outside on the playground before returning inside for lunch and nap. Today's lunch consisted of pizza, peaches, green beans and milk. All areas of the facility were inspected to include the playground area. The classroom off of the office is not being used by children at this time. There is age appropriate materials in each classroom. The first aid kit and non-medical supplies were inspected and found complete. The center had the required information posted. The inspector interacted with staff and children in care. Three staff records and five children's records were reviewed. The most recent evacuation drill was conducted on 03/14/2019. One of the two required shelter-in-place drills was documented on 03/20/2019. A second shelter-in-place drill will need to be completed by the end of the year. A lock down drill was completed on 03/21/2019. Management stated medication was not present during the inspection. Management stated transportation is not provided. Today's Ratios Observed: (2-5 years combined): 15 children with 3 staff. Infants: 4 infants with 1 staff. Toddlers: 3 toddlers with 1 staff. Description: Based on review of children's records, the facility did not obtain all required information for each child's record. Evidence: The record of child #1 did not contain the address for the two emergency contacts listed.

Plan of Correction: Per the director: I will contact the parent and have them complete the missing information.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not ensure all areas, inside and outside, were maintained in a clean, safe and operable condition. Evidence: A section of lattice along the fence is detached exposing a rough edge that could snag the skin of a child in care and has

Plan of Correction: Per the director: We will have the fence repaired over the weekend to reattach the lattice section.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on the review of the three records, the center did not request a search of out-of-state founded complaints of child abuse for one staff members who has lived outside of Virginia within the last five years. Evidence: 1. The record for Staff #2 (hired 03/11/2019) indicated the staff members had lived outside of Virginia within the last five years. 2. The records did not contain documentation of a request for founded complaints of child abuse from the state the staff members indicated they had lived in within the last five years. 3. Staff #2 was observed working at the center during the inspection. 4. Staff #1 stated an out-of-state child abuse search for Staff #2 had not been requested as of 04/19/2019.

Plan of Correction: Per the director: I will request an out of state founded complaints of child abuse for the staff member immediately. I will send documentation to the inspector within ten days.
