Comments:
Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on April 15, 2019 from 3:00 p.m. through 5:30 p.m. There were thirty-seven children in the care of four staff. Five children's records were reviewed. Five staff records were reviewed on April 23, 2019 at Boys and Girls Clubs office. No medication is being administered at this time. Areas of non-compliance may be found on the violation notice. Areas observed were: arrival, hand washing/bathroom procedures, free indoor play in the gymnasium, snack and homework. Interactions between the children and staff were observed as positive and the children seem comfortable with the daily routine and schedule. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on record review, there was not documentation for an updated tuberculosis screening for one staff. Evidence: 1. Five staff records were reviewed. 2. Staff #1 did not have documentation of obtaining an updated tuberculosis screening. 3. Last documented update was dated 11/01/2016.

Plan of Correction: Staff #1 will obtain an updated tuberculosis screening as soon as possible.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to obtain a complete physical address for one emergency contact. Evidence: 1. Five children's records were reviewed. 2. Child #2 did not have documentation of a complete physical address for the second emergency contact. 3. The director confirmed there was not a complete physical address for Child #2's second emerency contact.

Plan of Correction: The complete physical address will be obtained from the parent and documented.

Standard #: 22VAC40-185-500-A
Description: Based on observation, the center failed to have three children wash hands after eating snack. Evidence: 1. Fifteen children were observed eating snack. 2. Three children were observed not washing their hands after eating snack. 3. Staff confirmed the children did not wash hands after eating snack.

Plan of Correction: Staff will be reminded of hand washing procedures.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center failed to sanitized the tables before serving snack to children. Evidence: 1. Snack was observe for two groups of children. 2. Tables were not sanitized between serving group #1 snack and group #2 snack. 3. Staff confirmed the tables were not sanitized between serving snack to group #2.

Plan of Correction: Procedures for sanitizing tables will be reviewed with all staff.
