Comments:

An unannounced monitoring inspection was conducted on Monday, April 15, 2019 to determine the program's compliance with licensing standards. The inspector arrived at 3:40pm and concluded the inspection at 6:00pm. The census for today?s inspection consisted of 87 children in the direct care of 8 staff. The program?s director assisted the inspector throughout today?s inspection. Upon the inspector's arrival, the children were playing outside. Later, the children were observed entering the cafeteria after washing their hands and preparing to eat. After dinner, the children were observed taking bathroom breaks. The children were later separated into different groups. A group of children went outside, another group completed homework, and a third group participated in a creative/dramatics class. Staff interacted with children and parents in a positive manner. The areas where children receive care were inspected and found to be in compliance. The center had the following posted: license, daily schedule, emergency numbers, evacuation maps, menu and various parent information. The center's first aid kit and non-emergency supplies were inspected and found complete. Eight children's records and emergency contacts for staff on site were reviewed during today's inspection. Four emergency medications and authorizations were reviewed. Health Inspection: 02/27/2019 Fire Inspection: 12/13/2018 The last documented evacuation drill was held on 03/25/2019. The last shelter-in-place drill was documented on 09/12/2018. The monthly menu was posted and today's dinner consisted of: BBQ baked chicken, a roll, potato salad, fresh fruit, ranch dressing/ketchup, and milk. Description:	Based on a review of children's records and interview on 04/15/2019, the center did not maintain the required information for four out of eight children's records reviewed. Evidence: 1) The record for Child #1was missing the child's first date of attendance. 2) The record for Child #2 was missing the child's first date of attendance. 3) The record for Child #3 was missing the child's first date of attendance. 4) The record for Child #5 was missing the child's first date of attendance. 5) During interview, Staff #1 confirmed the dates of attendance were not on file for Child #1, Child #2, Child #3, and Child #5. Each center shall maintain first dates of attendance for each child enrolled.
Plan of Correction:	The licensing inspector and director reviewed attendance records while the inspector was on site and obtained the children's first date of attendance. Per the director: I will review each child's file new/old to ensure dates are written.
