The licensing inspector conducted an unannounced focused monitoring inspection on Tuesday April 9, 2019 from approximately 2:15 pm to 4:13 pm to determine the provider's compliance with licensing standards at the child day center. During this inspection, there were 34 children and 9 staff present including an administrative staff to assist as needed. The licensing inspector completed observations of the children in care napping in designated areas, transitioning from naptime, infants were eating their snacks, sitting at the table in the classrooms, interacting with peers and staff. There were three children's and four staff's records reviewed during this inspection. There were violations cited during this inspection. See violation notice for further details. Standard #: 22VAC40-185-70-A Description: Based on review of staff records and interview with staff, the center did not ensure to obtain all required information. Evidence: During the inspection on April 9, 2019, in Staff#3's record (date of hire: 4/08/19), the following required information was not found: 1. Verification of age requirement ad job title. 2. Written information to demonstrate that individual possesses the education, certification, and experience required by job position. Interview with staff confirmed an application or resume was not available in staff record for review during this inspection. Plan of Correction: The center will ensure that all applications and required information are on site.
Standard #: 22VAC40-185-340-D Description: Based on observation, review of staff records, and interview with staff, the center did not ensure in each grouping of children at least one staff member who meets the qualifications of a program leader shall be regularly present. Evidence: During the inspection on April 9, 2019, Staff#3 (date of hire 04/08/19) was observed in the classroom with four children present providing sole and direct care and supervision. Interview with staff reported being lead teacher qualified but the required information was not available in the record for further verification. Plan of Correction: The center will ensure transcripts, training, education to verify staff qualifications are included in record on site.
Standard #: 22VAC40-191-60-C-2 Description: Based on review of staff records and interview with staff, the center did not ensure to obtain central registry findings within 30 days of employment or volunteer service. Evidence: During the inspection on April 9, 2019, in Staff#2's record (date of hire 3/04/19) the results for a central registry finding was not found. Interview with staff confirmed the results were not available during the record review and the last follow up by the center was made on March 28, 2019. Plan of Correction: The center will ensure to follow up weekly and within 30 days plan to resend.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on review of staff records and interview with staff, the center did not ensure to obtain a copy of the results of an out of state search of the central registry for required staff. Evidence: During the inspection on April 9, 2019, in Staff#2's record (date of hire: 03/04/19) a copy of results for central registry searches for the states of Texas and Louisiana were not found. Interview with staff confirmed the requests for these out of states results for central registry was not completed and results were not available. Plan of Correction: The center will follow today with other states to obtain finding results.
