Comments:
Thank you for your time and assistance with the unannounced monitoring inspection conducted today, April 22, 2019. The on-site inspection was from 10:30a.m. to 3:45p.m. On this date, at the time of arrival, there were 37 children present in four classrooms and in the care of seven staff members, with one extra staff member available to assist, if needed. There were 53 children enrolled in the program. Six staff members' records and five children?s records were reviewed. All four classrooms were toured and observations were completed in two of those rooms. The medication and medication paperwork were reviewed for three children. There were six staff members and four children interviewed. The exception to compliance was noted on the violation notice and the risk rating was noted on the supplemental page. The the current themes in two of the classrooms were Plants/Flowers, Spring and Insects. Lunch was observed to meet the USDA guidelines and consisted of roasted chicken, rice, peas, fresh oranges and milk. Some items observed and reviewed during the inspection included: outdoor play, choice time, center time, music and movement, hand washing and bathroom procedures, transitions, behavioral guidance, lunch, nap time, afternoon snack, story time, medications, emergency evacuation, shelter-in-place, and lockdown drills, required posted information, injury reports, first-aid kit and emergency supplies, annual health inspection, and warm water temperature. Standard #: 22VAC40-185-510-C
Description: Based on review of the medications and medication paperwork for three children and confirmation from the director, one child had been administered a medication that was beyond it's expiration date. Evidence: 1. One prescription medication on-site for Child C had an expiration date of February 2019. The medication was administered to Child C on 04/02/2019.

Plan of Correction: The director has been in contact with the parent of the child and will obtain new medication as soon as possible. In the future, staff will check medications once a week to ensure neither the medications or the authorizations have not expired.
