An unannounced monitoring inspection was conducted from 9:00am-11:30am. Upon arrival there 130 children in care with 27 staff present. A sample of 10 children and 2 staff records were reviewed, in addition to 3 over-the-counter skin products. Children were observed on the playground, during circle time, playing in centers and practicing for the school play.
Standard #: 22VAC40-185-270-A Description: Based on observation the center did not ensure areas and equipment of the center were maintained in a safe condition. Evidence: In the two's bathroom one of the toilets had a 2 inch crack on the seat. The edges were jagged, exposing the non -porous surface underneath. Plan of Correction: The following corrective action was provided by the program director: A new toilet will be ordered today and replaced this week.
Standard #: 22VAC40-185-280-B Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place using a safe locking method. Evidence: In the main preschool classroom, the supply closet was not locked. There was a bottle of contact solution inside the closet on the bottom shelf. Plan of Correction: The following corrective action was provided by the program director: The current safety latch did not lock properly. New latches were ordered.
Standard #: 22VAC40-185-520-C Description: Based on a record review the center did not ensure all requirements when diaper cream us used. Evidence: There were 2 diaper ointment authorizations in the main preschool room that were expired. Plan of Correction: The following corrective action was provided by the program director: Both diaper creams were removed from the classroom.
