An unannounced monitoring inspection was conducted from 7:15 a.m. - 8:45 a.m. There were 30 children in care being supervised by 4 staff members. The children were observed participating in table top activities, cooperative games, and preparing for breakfast. Breakfast served today is turkey sausage bagel sandwich, cereal bar, cereal with milk, and fruit juice. Ten children's records were reviewed. Four staff records were reviewed and in compliance. Eight medications were reviewed and in compliance. There are currently two staff members on site who are MAT (Medication Administration Training) certified. The most recent emergency escape drill was conducted 03/29/2019 and the most recent shelter in place drill was conducted 09/17/2019. Evidence: Child A (start date: 08/27/2018) did not have documentation of immunizations on file. Plan of Correction: Administration will request a copy of the most recent immunization record from the parents of child A.
Standard #: 22VAC40-185-140-A Description: Based on review of 8 children's records it was determined that not all files contained documentation of physical within the first 30 days of the child's enrollment. Evidence: Child A (start date: 08/27/2018) did not have documentation of a physical on file. Plan of Correction: Administration will request a copy of the most recent physical from the parents of child A.
