Comments:
Today an unannounced Renewal Inspection was conducted between the hours of 9:10am and 1:00pm. There were 6 classrooms observed with a total of 41 children with 8 staff within the supervision guidelines. The children were observed eating breakfast (cereal and fruit with milk) singing and dancing, making flowers out of pipe cleaners and other organized activities. A complete inspection of the physical plant, children and staff records, fire drill log, medication and emergency procedures were observed during this inspection. Five medications and authorizations were reviewed. Positive interactions between children and staff were observed. Standard #: 22VAC40-185-130-B
Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: Child #5 (date of birth: 1/9/2018) most recent immunizations are dated 3/9/2018.

Plan of Correction: I'll email the parents and let them know that I'll need updated records as soon as possible.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain documentation of a physical exam within the first 30 days of attendance for each child. Evidence: Child #4 file does not contain a physical exam.

Plan of Correction: I'll email and talk to the parents today and let them know I need this as soon as possible.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all required information. Evidence: Child #2 and Child #4 files did not contain a second emergency contact. Child #3 file did not contain an address for their second emergency contact.

Plan of Correction: I will contact the parents today and I'll have the parents fill in the missing contact information.

Standard #: 22VAC40-185-280-B
Description: Based on observation, hazardous substances were not kept in a locked place using a safe locking method that prevents access by children. Evidence: There are various cleaning materials stored in the closed of the transitional kindergarten classroom. There is a lock on closet door knob and the keys are hanging in the door knob making the cleaning materials accessible to the children.

Plan of Correction: I will talk with this staff member and I'll start doing daily checks to make sure doors are locked.

Standard #: 22VAC40-185-510-C
Description: Based on a review of medications, the center's procedures for administering medication was not followed. Evidence: Child #5 emergency medication expired 1/2019.

Plan of Correction: I've already emailed mom to let her know the medication is expired and I'll send it home tonight at pick-up.
