Comments:
The licensing inspector conducted an unannounced inspection in response to a complaint that was received in the licensing office on 04/01/19, regarding allegations in the area of Special Care Provisions. Interviews were conducted with staff , records were reviewed and observations of the program were made relating to the allegation. The information gathered during the investigation supports the allegation, resulting in a violation cited.

Complaint related: Yes
Description: Based on observation, documentation review and interviews with staff, the center did not ensure all requirements were met when diaper ointment cream was used. Evidence: On 04/01/19, child #1 had a current written authorization from the parent to apply diaper ointment, as needed for a rash. Staff and administration confirmed child #1 had a visible diaper rash and was asked by the parent to change child #1 every hour. The center did not have a record of when diaper cream was applied with any adverse reactions during diaper changes.

Plan of Correction: During the exit interview the following corrective action was provided by the director: All staff were counseled regarding over-the-counter skin products and were retrained on documentation of use. Management will monitor for future compliance.
