An unannounced monitoring inspection was completed on 04/24/2019. There were 22 children were in care at the center, ages two years through six years old with four staff supervising the children. The inspectors arrived at the program at 9:30 A.M. and departed the program at 12:00 P.M. Five staff's records and six children's records were reviewed. Informal interviews were held with all staff members and there were interactions with most of the children. A walk-through of the center and house was done. The children were observed in an inside activity, dance time, free play, outside time and snack. Discussion with staff about: staff record keeping and labeling lunch boxes. The inspectors also inquired about scheduling and closures of the center.

Standard #: 22VAC40-185-60-A Description: Based on review of six children's records, the center failed to ensure complete records according to standards. Evidence: 1. File for Child 1 was missing proof of identity. 2. File for Child 4 was missing Chronic Physical Problems in the enrollment form. Plan of Correction: The Program Lead stated that they would obtain and monitor children's records.
Standard #: 22VAC40-185-560-G Description: Based on observation, the center failed to ensure that when food was brought from home, food was not labeled according to standard. Evidence: 1. Observed all lunch boxes for twenty two children without owner's name. Plan of Correction: The program leader will label all lunch boxes today and staff will monitor daily.
