Comments:
An unannounced renewal inspection was completed on 04/29/2019. There were 18 children, ages 6 weeks-4 years, in the care of 5 staff members during the inspection. The inspector reviewed 5 children's records, 5 staff record and the record for the facility owner during the inspection. The children were observed in the following activities: during free choice play/center play, having lunch and during nap time. Infants were observed being fed and allowed to sleep on demand. Proper hand washing and table sanitization were also observed during the inspection. The inspector reviewed the emergency preparedness plans, the injury prevention plan, the playground safety plan, the sick child policy, and the emergency supplies during the inspection. The inspector discussed the following with the staff: over-the-counter skin products, model forms, the renewal process, the playground, and position qualifications. Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards. Evidence: 1. The record for Child #1 did not contain the following information: the work phone number and place of employment of each parent who has custody (there was a place of employment listed however, there was no work phone number provided) and the health information as required by 22VAC40-185-130 (there was not an immunization record in the record for Child #1. 2. The record for Child #3 did not contain the following information: the work phone number and place of employment of each parent who has custody (there was a place of employment listed however, there was no work phone number provided) and the name, address and phone number of two designated people to call in an emergency if a parent cannot be reached (the address was incomplete for the second contact listed). 3. The record for Child #5 did not contain the work phone number and place of employment of each parent who has custody (there was a place of employment listed however, there was no work phone number provided).

Plan of Correction: The Program Director will get the information.

Standard #: 22VAC40-185-500-B
Description: Based on observations and discussion with the staff, the facility failed to ensure that a nonabsorbent surface for diapering or changing shall be used. Evidence: The changing tables in both the Toddler Room and the Two Year Old Room had mats on them where the children are laid on to be changed. The mats on both of the changing tables were worn and the absorbent portion of the mat was exposed.

Plan of Correction: The Program Director removed them during the inspection and they will be replaced.

Standard #: 22VAC40-185-520-A
Description: Based on observations and discussion with the staff, the facility failed to ensure that over-the-counter skin products shall not be kept beyond the expiration date of the product. Evidence: There was a container of Vanicream on-site for Child #2. The expiration date on the Vanicream was 03/2019.

Plan of Correction: The Program Director disposed of the cream during the inspection. Moving forward all dates will be checked monthly.
