Standard #: 22VAC40-185-130-A Description: Based on review of five children's records and verification from the director, an immunization record for one child was not obtained before the child attended the center. Evidence: 1. The record for Child #5 contained an immunization record and documentation that the child was adequately immunized dated 01/05/2017. Date of enrollment for Child #5 was 09/08/2016. 2. The director confirmed that an immunization record had not been obtained before the child started at the center. Plan of Correction: The director will ensure that an immunization record is on-site before any child attends the center.
Standard #: 22VAC40-185-140-A Description: Based on review of five children's records and verification from the director, one child's record did not contain documentation that the child had received a physical examination within one month of attendance. Evidence: 1. The record for Child #5 contained documentation that the child had received a physical examination dated 01/20/2017. Date of enrollment for Child #5 was 09/08/2016. 2. The director confirmed that the physical exam report was received after the child had been enrolled for over a month. Plan of Correction: The director will ensure that a physical examination report is obtained for every child within 30 days of enrollment.
Standard #: 22VAC40-185-500-A Description: Based on observation and discussion with staff, two children in one classroom failed to wash their hands after eating snack. Evidence: 1. Two children in Classroom C finished their snack and proceeded to go to the rug to listen to a story being read without going to wash their hands first. 2. Staff #4 confirmed the two children did not wash their hands after eating snack. Plan of Correction: The director will remind all staff that all children must wash their hands both before and after eating.
