Comments:
A renewal inspection was conducted with the director and three staff. There were 31 children present during the inspection and the age range was five years to ten years old. The children were observed during homework time, snack and centers. Two staff are current in MAT. Eight children's records were reviewed. Violations:
Standard #: 22VAC40-185-240-C
Description: Based on review of four staff records not all had documentation of 16 hours of annual training. Evidence: Staff A had documentation of six hours of annual training training.

Plan of Correction: Staff stated the training have been completed however the documentation could not be found.

Standard #: 22VAC40-185-550-E
Description: Based on review and staff interviews the record of the escape practices and shelter in place could not be found. Evidence: The record of the monthly and escape practices and the shelter in place drills could not be found.

Plan of Correction: We will keep looking for it.
