Standard #: 22VAC40-185-160-C Description: Based on review of staff records, the center failed to ensure that an updated tuberculosis screening was completed at least every two years from the date of the last documented test or screening. Evidence: 1. Staff #2's last documented tuberculosis screening was dated 11/10/2016. 2. Staff verified that the tuberculosis screening dated 11/10/2016 was the most current one they had on file. Plan of Correction: Administration will have the staff member set up an appointment this week.
Standard #: 22VAC40-185-280-B Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location. Evidence: 1. In the School Age Classroom Restroom there was a can of Dollar General Disinfectant Spray in a cabinet above the toilet. 2. The warning label stated "keep out of reach of children, flammable, and chemical hazards." 3. Staff verified that there was a can of Dollar General Disinfectant spray in an unlocked cabinet. Plan of Correction: Staff immediately moved the disinfectant spray to a locked location. The administrator will review the hazardous substance material requirement with all staff.
Standard #: 22VAC40-185-550-M Description: Based on review of injury reports, the center failed to include all required information on the injury reports. Evidence: 1. One accident report dated 03/11/2019, two accident reports dated 02/19/2019, and one accident report dated 03/29/2019 were missing the preventive action for reoccurrence. 2. Staff verified that the reports were missing the preventive action for reoccurrence completed. Plan of Correction: Administration will review the procedures for completing accident reports will all staff.
