Comments:
An unannounced monitoring inspection was done today 04/29/2019 beginning at 8:00 AM and ending at 12:00 PM. There were 53 children present. Five children's and nine staff's records were reviewed during the inspection. Outside time, breakfast, playtime, Lunch and nap were observed, policies and procedures and a physical plant inspection was completed.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on review of five children's records, the center failed to ensure records were complete. Evidence: Child 2 was missing father's contact information, previous care in other programs, and chronic physical problems and allergies. Child 3 was missing doctor's name and phone number.

Plan of Correction: Staff will ensure that missing information and monitoring will take place.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure all areas and equipment were safe, clean and operable. Evidence: 1. Observed in the toddler playground, the teal and pink climbing equipment had visible cracks that posed snagging risk to children. 2. Observed in the main playground area, the border pieces had deteriorating wood which posed a trip hazard to children and staff. 3. Observed near an exterior door, standing water accumulated on plastic baby pools and water tables.

Plan of Correction: Staff will ensure that the playground and exterior are monitored for any safety concerns. Staff will repair and/or replace any equipment needed. Staff will also monitor.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to store hazardous substances in an inaccessible and locked place. Evidence: Room air freshener spray was stored on a high cubby (in the pre-k room).

Plan of Correction: Staff will place all chemicals in locked closets.

Standard #: 22VAC40-185-330-B
Description: Based on observation, the center failed to ensure that the resilient surfacing for all climbable and moveable equipment was met. Evidence: 1. Directly under the swings, there was no measurable resilient surfacing. Children were observed playing on the swings. 2. Observed in the toddler area, under the climbable equipment, there was no measurable resilient surfacing. No children were observed playing in this area.

Plan of Correction: Staff will order additional resilient surfacing and continue to monitor.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center failed to ensure that all diapering requirements were met according to standards. Evidence: 1. The diapering pad was no longer nonabsorbent due to tears in the changing pad. 2. The foot operated trash can did not open wide enough to prevent the soiled diaper from touching the trash can surface.

Plan of Correction: Staff will replace changing pad and trash can and monitor for any malfunctioning.
