Comments:
An unannounced renewal inspection was conducted with center staff to determine compliance with the licensing standards. There were 75 children present, ranging in age from 2 months to five years, with ten staff actively supervising during rest. After rest, the children had afternoon snack and played outdoors. Snack served today: rice krispie treats and juice. Mulch has been ordered to replenish the playground. Four emergency medications for three children were reviewed. Seven children's files and three staff files were reviewed - please see Supplemental Summary for details. The last health inspection was conducted 9/18/2018. The last fire inspection was conducted 3/21/2019. Evacuation drills have been conducted monthly; the last shelter in place drill was conducted 4/12/2019. Violations:
Standard #: 22VAC40-185-90--A
Description: Based on review of seven children's files and staff interview, the facility failed to have a written agreement between the parent and the center in each child's file by the first day of attendance to include: a statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately. Evidence: the statement was not included on signed agreement in seven children's files.

Plan of Correction: Licensing inspector will send the VDSS model form that includes the policy. The facility will include the statement in the signed agreement with parents.

Standard #: 22VAC40-185-350-E-2
Description: Based on observation, the facility failed to maintain the required staff to child ratio requiring one staff for every five children, ages 16 months old to two years. Ratios may be doubled during rest time. Evidence: there were twelve children ages 16 months to two years in a classroom with one staff during the designated rest period.

Plan of Correction: Center administration will be mindful of the doubled ratios during rest time.

Standard #: 22VAC40-185-510-A
Description: Based on review of four children's medications, the facility failed to prevent the use of an expired medication. Evidence: the emergency medication for one child expired 1/2019, The medication was administered in 3/2019.

Plan of Correction: Assistant director will return the medication to the parent today.
