Comments:
The licensing inspector conducted an unannounced inspection on Thursday, April 4, 2019 from 10:45am to 12:40pm in response to a self-reported incident that the Department received on April 3, 2019 regarding special feeding needs. The inspector completed a follow up inspection on April 11, 2019. Interviews with staff were completed, video footage was reviewed, and documentation was reviewed. The preponderance of the evidence gathered during the investigation validates the self-reported incident. Violations were cited. See the violation notice for more details. Violations:
Standard #: 22VAC40-185-370-5
Description: Based on a a review of video footage on 04/04/2019, the center did not ensure that awake infants not playing on the floor or ground a change in play space at least every 30 minutes. EVIDENCE: 1. According to video footage taken on 04/03/2019, Child #1was in the high chair from 2:59pm to 4:20pm. 2. On the video footage Child #1was observed eating snack from 2:59pm to 3:20pm. 3. On the video footage Child #1 was observed in the high chair from 3:20pm to 4:20pm.

Plan of Correction: Immediate training of infant teachers to reinforce the fact that awake infants not playing on the floor or ground, must have a change in play space at least every 30 minutes. ? Identify different play spaces in each of the infant rooms. ? Rotate children who are not moving on the ground and not asleep every 30 minutes to a different play space or more frequently as determined by the individual infant?s needs. ? To develop a routine and consistency, the times to check/rotate would be on the hour and at the half hour. Review training at next staff meeting for all employees.

Standard #: 22VAC40-185-570-A
Description: Based on a review of video footage and documentation on 04/04/2019, the center did not ensure that when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. EVIDENCE: 1. According to video footage from 04/03/2019, Staff #1 placed Child #1 in a high chair. Staff #1 did not strap Child #1 in the high chair. 2. Video footage from 04/03/2019 shows at 4:20pm Child #1 pull himself over the side of the high chair and fall to the floor. Child #1 landed on his back. He also bumped his head when he landed. 3. An incident report dated 4/3/2019 states that Child #1 got a bump on the top of his head.

Plan of Correction: 1. Immediate training of teachers who place children in seating with straps. ? High chairs, infant carrier seats, or feeding tables shall be used for children under 12 months who are not held while being fed. ? Children using infant seats or high chairs shall be supervised during snacks and meals. ? When a child is placed in an infant seat or high chair, the protective belt shall be fastened securely. ? The administration will randomly visit infant / toddler rooms to ensure that all children in seating devices are secured properly. 2. Review training at next staff meeting for all employees.
