Comments:
The licensing inspector conducted an unannounced monitoring inspection Tuesday, April 30, 2019 from 10:05am to 12:35pm. The children were observed playing in the learning centers. A formal observation was completed with this group. Later the children were observed cleaning up, playing on the playground, and later eating lunch. Lunch today consisted on taco meat, tortillas, corn, beans, fruit and milk. Five children's records and four partial employee records were reviewed during this inspection. The center's first aid kit and emergency supplies were inspected. Last emergency drill: 04/29/2019 Last shelter-in-place drill: 03/20/2019 Last fire inspection: 04/01/2019 Last health inspection: 10/05/2018 Total enrollment: 18 Today, the following child to staff ratios were observed: Preschool (3-5 years old)-18:2 The violations from previous inspections were checked for corrections. Standard #: 22VAC40-185-140-A
Description: Based on a review of children's records on 04/30/2019, the center did not ensure that each child has received a physical examination before the child can attend the center or within one month after attendance. EVIDENCE: The physical in the record of Child #4, whose start date at the center is 08/04/2018, is dated 02/28/2019.

Plan of Correction: Physical dated 2/28/2018 was received, however it can not be located at the moment. Will make sure to keep original physical in the teacher's folder kept at the center.

Standard #: 22VAC40-185-540-C
Description: Based on observation on 04/30/2019,the center did not ensure that the required first aid kit contained the minimum items. EVIDENCE: The first aid kit on the bus did not include the following items: scissors, tweezers, thermometer, gauze tape, cold compress.

Plan of Correction: Will have assistant that rides the bus take the first aid kit from the center with her.
