Comments:
An unannounced Renewal Study was conducted from 7:10 a.m. - 8:25 a.m. There were 52 children in care being supervised by 6 staff members. The children were observed participating in table top activities and cooperative games. Breakfast served today is ham and cheese croissant, fruit juice, baby carrots, and milk. Ten children's records were reviewed and in compliance. Six staff records were reviewed. Six medications were reviewed. The most recent emergency escape drill was conducted 04/01/2019 and the most recent shelter in place drill was conducted 11/29/2018. Description: Based on review of six staff records it was determined that not all records contained documentation of a TB (tuberculosis) screening every two years. Evidence: The most recent TB screening on file for Staff A was dated 11/09/2013.

Plan of Correction: Staff A will obtain a TB screening and submit results to administration.

Standard #: 22VAC40-185-240-D-1
Description: Based on review of six staff records and staff interview it was determined that the center failed to ensure there was a staff member on site with MAT (Medication Administration Training) during the center's hours of operation. Evidence: Six medications were reviewed however, there were no staff members on site with current MAT certification.

Plan of Correction: Administration will schedule a MAT class for staff on site.
