Comments:

A Subsidy Health and Safety inspection was conducted today from 9am to 11:45am to review supplemental health and safety requirements. There were 34 children under the supervision of 8 staff. Children were observed participating in circle time, singing songs, playing individually and in groups, interacting with staff, arriving to care, and getting ready to go outside to play. There were medications on site today. There was a sufficient supply of books, toys, and materials that were developmentally appropriate for the children in care. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Description:	Based on review, the facility did not ensure that within 90 days of employment all staff who work directly with children shall complete the Virginia Preservice Training for Child Care Staff. Evidence: Reviewed 5 staff records and found that Staff 3 did not have documentation of completing the Virginia Preservice within 90 days of employment.
Plan of Correction:	Will complete by the end of May.

Description:	Based on review, the facility did not ensure that staff obtain CPR training within 90 days of employment. Evidence: Reviewed 5 staff records and found that Staff 3 and 4 did not have documentation of CPR training and have been employed for more than 90 days.
