An unannounced renewal inspection was conducted today from 10:50am-2:00pm. There were 64 children (infants-5yrs.) directly supervised by 11 staff. The physical plant, 5 staff records, 5 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, being bottle fed, listening to a story, participating in music class, and playing outside. There was an abundant supply of materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified on the Violation Notice. Standard #: 22VAC40-185-130-B Description: Based on record review, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: Child#3 contains an immunization record that was last updated on 10/20/2018. On the day of their last immunization the child was 7 months of age. Child#4 contains an immunization record that was last updated on 8/21/2018. On the day of their last immunization the child was 15 months of age. Plan of Correction: Corrected during the inspection
Standard #: 22VAC40-185-510-C Description: Based on review of medication, the center's policies when addressing long term medication and outdated medication were not followed. Evidence: Child#6 has a long term medication on-site that expired on 01/31/2019, the medication was not returned to the parent to obtain a current medication. Plan of Correction: corrected during the inspection.
Standard #: 63.2(17)-1720.1-B-3 Description: Based on staff record review, there is no documentation that Child Abuse and Neglect checks were obtained for an employee that has resided outside the state of Virginia within the last 5 years. Evidence: Staff#1 (DOH: 12/21/2017) has resided outside the state of Virginia within the last 5 years, their records did not have documentation that out-of-state child abuse and neglect results were obtained. Plan of Correction: Corrected during the inspection.
