Please complete http://www.dss.virginia.gov/facility/injury_report_subsidy.cgi to report any serious injury to the Division. Serious injuries are defined as an injury sustained while in day care that resulted in the child getting outside medical treatment.

Comments:
An unannounced Subsidy Health and Safety Inspection was conducted on May 1, 2019 to review supplemental health and safety requirements. A licensing inspection was also conducted on this date (see separate report for details). Only subsidy requirements that are not covered in the licensing standards are reviewed during this supplemental inspection. There were 26 children present ranging in age from 14 months to 2 years old. All current staff have documentation of the mandatory VA Pre-service Training and are currently certified in first aid and CPR. New staff have 90 days from hire to complete the training. There is one child with a diagnosed food allergy with the required Allergy Action Plan. Reminder: evacuation drills are required monthly, shelter in place drills are required twice a year, and a lockdown drill is required once a year. Lockdown drills are to isolate children and staff from a security threat; shelter in place is to protect children from a natural or manmade threat. The last lockdown drill was conducted 1/25/2019.
