An unannounced non-mandated monitoring inspection conducted on April 11, 2019. There was 52 children and 10 staff present. Additional administrative staff was available to assist. The children were observed singing songs , washing hands, outside playing and variety of table activities. The facility focus inspection was in standard requirements for physical plant, children records , staffing and supervision, emergency preparedness plan and injury reports.

Standard #: 22VAC40-185-140-A Description: Based on 1 of 5 records reviewed , the licensee is not ensure that each child had a physical examination before the child's attendance or with in one month after attendance. Evidence: During the inspection conducted on April 11, 2019, there was no documentation of a physical examination the record for child #5 ( start date 12/03/2018). Plan of Correction: Director contacted parent about the physical . The parent will have the physical to the center today.
Standard #: 22VAC40-185-60-A Description: Based on 1 of 5 children's records reviewed , the licensee did not ensure each child enrolled has a record which contained all required information. Evidence: During the inspection conducted on April 11, 2019, the record for child #3 did not have two emergency contacts. Plan of Correction: Director contacted parent. Parent will provide the information.
