Comments:
An unannounced monitoring inspection was conducted on Thursday, April 18, 2019 to determine the center's compliance with licensing standards that could not be determined during the initial inspection. The inspector arrived at the center at 10:30am and concluded the inspection at 4:00pm. The center's director assisted the inspector throughout the inspection. The census for today's inspection consisted of 116 children. The children were observed during a variety of activities to include playing with toys in their designated classrooms, playing in the playground, completing art activities, and reading books. The staff were observed having positive interactions with the children. Five children's records, four medication records and medication administration, two staff's records, and 34 background checks for staff were reviewed. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of five children's records and interview on 04/18/2019, the center did not obtain documentation that one child had been adequately immunized before the child attended the center. Evidence: 1) The record for Child #1 (DOE: 02/25/2019) did not contain an immunization report. 2) During interview, Staff #18 confirmed the immunization report for Child #1 was not in the child's record at the time of the inspection.

Plan of Correction: Per the Center: The immunizations and physical were previously in the child's record, but were lost during the change in ownership. The parents have been contacted and requested to provide the missing information.

Standard #: 22VAC40-185-140-A
Description: Based on a review of five children's records and interview on 04/18/2019, the center did not ensure one child obtained documentation of a physical examination by or under the direction of a physician prior to attendance or within one month after attendance. Evidence: 1) The record for Child #1 (DOE: 02/25/2019) did not contain documentation of a physical examination on file. 2) During interview, Staff #18 confirmed the physical examination report for Child #1 was not in the child's record at the time of the inspection.

Plan of Correction: Per the Center: The immunizations and physical were previously in the child's record, but were lost during the change in ownership. The parents have been contacted and requested to provide the missing information.

Standard #: 22VAC40-185-70-A
Description: Based on a review of two staff's records and interview on 04/18/2019, the center did not obtain the required information for one staff member. Evidence: 1) The record for Staff #1 (DOH: 04/09/19) did not contain documentation of at least two character references prior to the staff beginning employment. 2) During interview, Staff #18 stated that the references were obtained over the telephone, but not recorded as required. If a reference check is taken over the phone, documentation shall include: dates of contact; names of persons contacted; the firms contacted; results; and signature of person making the call.

Plan of Correction: Per the Center: In the future, when references are taken over the telephone, they will be documented as required.
