Standard #: 22VAC40-665-540-A Description: Based on vendor interviews, the vendor did not ensure all staff shall be evaluated by a health professional and be issued a statement that the individual is determined to be free of communicable tuberculosis (TB). Documentation of the screening shall be submitted at the time of employment and prior to coming into contact with children. The documentation shall have been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department. Evidence: During the inspections the following records did not have TB screenings as required. 1) The record for Staff #1 and Staff #2 did not include a TB screening available for review. Center Management confirmed Staff #1 and Staff #2 began working in the center on 3-18-2019. 2) The record for Staff #3 did not include a TB screening within the required time frame. The TB screening for Staff #3 available for review was dated 6-23-17. Staff #3 began working in the center on 3-18-2019. 3) The record for Staff #4 did not include a TB screening within the required time frame. The TB screening for Staff #4 available for review was dated 4-16-19. Staff #4 began working in the center on 4-15-2019. 4) The record for Staff #9 did not include a TB screening within the required time frame. The TB screening for Staff #9 available for review was dated 6-5-17. Staff #9 began working in the center on 11-28-2019. Staff #1, Staff #2, Staff #3, Staff #4 and Staff #9 did not have documentation of a TB screening within the last 30 days and prior to coming into with children. Plan of Correction: The Center Management will ensure staff will get new TB screenings and in the future will make sure verify they have TB screening before they start working.
