The provider must send documentation to the LI that the background checks have been requested no later than 10 days following this notification dated 05/03/19. The following noncompliance of background checks requirements have been identified during today?s inspection: 191 -C2
- Comments:

An unannounced monitoring inspection was conducted from 8:30am to 10:00am. At the time of the tour there were 19 children in care with 4 staff present. A sample of 2 children and 3 new staff records were reviewed. There were no food allergies or medications to review. A tour of the cafeteria and gym, which is occasionally used was given by the program director. Children were preparing to be escorted to their classrooms for the regular school day.

Standard #: 22VAC40-185-280-B Description: Based on observation the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children. Evidence: There was an air freshener on the bathroom floor next to the toilet. The solid chemical portion of the freshener was exposed and in reach of the children. There were 2 liquid plug-ins air fresheners observed in the electrical outlets in the wall. Plan of Correction: The following corrective action was provided by the program director during the exit interview: The director will remove all air fresheners from the classroom and bathroom areas.
Standard #: 22VAC40-191-60-C-2 Description: Based on a record review, the center did not ensure all staff had the required background checks. Evidence: Staff #2 (hired 02/25/19), did not have documentation of the results of a central registry check. Plan of Correction: The following corrective action was provided by the district manager: Staff #2 central registry had been sent off in March. She will call and follow-up on the status of this check today.
