Comments:
An unannounced monitoring inspection was conducted from 9:00am- 11:00am. Upon arrival there were 22 children in care with 4 staff present. A sample 2 staff and 3 children's records were reviewed. There were no children in care at the time of inspection that required medication. The main school campus was clsoed for prom, however the day care remained open. Children were observed on the playground.

Standard #: 22VAC40-185-40-K
Description: Based on a record review, the center did not have documentation of all required components of playground safety procedures. Evidence: The center's playground policy did not include, provision for active supervision by staff to include positioning of staff in strategic locations, scanning play activities, and circulating among children, and maintaining surfaces.

Plan of Correction: The following corrective action plan was provided by the program director: The director will be working on the playground policies to include all required procedures.
