Comments:
A joint investigation was conducted with the local agency on February 28, 2019 from 10:30 a.m. to 12:00 p.m. regarding a complaint of forbidden action- rough handling, supervision and medication. Staff interviews were conducted. Documentation was reviewed. Based upon the information gathered during the investigation, the complaint regarding forbidden action, supervision and medication could not be substantiated. Description: Based on review of staff records, the center failed to ensure that staff received orientation training by the end of their first day of assuming job responsibilities. Evidence: Staff #1, start date 9/6/2018, orientation training was dated 9/11/2018 and Staff #2, start date 2/11/2019, did not have documentation of orientation training in the record.

Plan of Correction: Staff #2 did receive orientation training on 2/11/19 but the checklist was not signed. Signatures have been documented

Description: Based on staff interviews and review of staff records, the center failed to ensure there was at least one staff member who met the qualifications of program leader for each grouping of children. Evidence: On February 14, 2019, three staff members were assigned to the 'closing' classroom with children ranging in age from two years to eight years of age. Of the three staff members, Staff #1 was under the age of 18 years old, Staff #2 had 3 days of programmatic experience at the center and Staff #3 had 14 days of programmatic experience at the center. None of the assigned staff met the requirement for the exception at the last hour of operation that there is a staff member in the group who is over 18 years of age and has at least three months of programmatic experience at the center.

Plan of Correction: Only staff that have 3 months plus experience at the center and are over the age of 18 years will be in the class to abide by the standard.
