Technical Assistance:
The climbing structure on the right side of the playground has been marked off-limits by the facility due to maintenance needs.

Comments:
An unannounced monitoring inspection was conducted beginning at 10:20 a.m. and ending at 1:00 p.m. with the assistant director. There were 43 children in care ranging from age 2 years through 5 years of age with six staff. Two year olds were observed having outdoor play on the bike track. Three year olds were observed having play time on the playground. Preschoolers were observed during large group activity and transition outside for outdoor play. Outdoor play areas were observed. After outdoor play, children returned inside to prepare for lunch. Lunch today was macaroni and cheese, corn, mixed fruit and milk. Six children's records and four staff records, including one new staff were reviewed. Nine medications, authorizations and an administration log were reviewed. Standard #: 22VAC40-185-70-A
Description: Based on review of three staff records, the center failed to obtain references before the first day of employment. Evidence: Staff #4's record, start date 4/29/2019, did not have documentation of two references regarding character, reputation and competency before employment.

Plan of Correction: References were obtained during the inspection.

Standard #: 22VAC40-185-240-D-1
Description: Based on review of nine medications, the center failed to ensure that staff completed the required training program for all medications on site. Evidence: There is prescription Diastat for Child #3 on site and no staff has completed the required Medication Administration Training for rectal medication.

Plan of Correction: Staff #2 completed the required training during the inspection

Standard #: 22VAC40-185-330-B
Description: Based on observations, not all resilient surfacing met the minimum requirements. Evidence: On the Two's playground, there was dirt observed under the composite climbing structure slide and the metal lip of the metal and wood structure. Resilient surfacing should be at least six inches deep.

Plan of Correction: Staff will be reminded to ensure that the pea gravel remains full at use and fall zones.

Standard #: 22VAC40-185-510-C
Description: Based on review of nine medications and authorizations, the center failed to follow it's medication policy. Evidence: Child #3 has Motrin on site with an authorization signed by the parent dated 11/27/2018. Shorterm medication authorization shall be no longer than 10 work days. Longterm medication requires an authorization from the parent and physician.

Plan of Correction: The authorization will be updated today.

Standard #: 22VAC40-185-510-G
Description: Based on review of nine medications, the center failed to ensure medication authorization requirements were met. Evidence: Child #2 has authorization from a physician for Auvi-Q and Benadryl but there is not written authorization from the parent or guardian.
