Technical Assistance:
Technical assistance was provided regarding storage of diaper creams and medications. The renewal process was discussed. Please submit your renewal application as soon as possible.

Comments:
An unannounced renewal inspection was conducted today from 11:00am-2:15pm. There were 39 children directly supervised by 6 staff and all classrooms were in ratio. The physical plant, 5 staff records, 4 children?s records, 3 medications, the playground, evacuation drills, emergency supplies, and policies were inspected. Children were observed reading books, playing outside, coloring, and infant care was observed. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-160-A
Description: Based on record review, not all staff have submitted documentation of a negative tuberculosis screening within 21 days after employment or a screening that was completed within 12 months prior to employment. Evidence: 1. Staff #2 (Date of Hire: 5/14/18) does not have a tuberculosis screening on file.

Plan of Correction: I will get a copy of the TB test by tomorrow.

Standard #: 22VAC40-185-160-C
Description: Based on record review, not all staff have documentation of updated tuberculosis screening or test completed within the last two years. Evidence: 1. Staff #1's most recent tuberculosis test on file was completed on 3/19/16. 2. Staff #4's most recent tuberculosis test on file was completed on 1/24/17.

Plan of Correction: The 2 staff will get updated TB tests by next week.

Standard #: 22VAC40-185-60-A
Description: Based on record review, not all children's records were complete. Evidence: 1. Child #3 and Child #4's records were missing 1 of 2 addresses for emergency contacts. Child #3's first day of attendance at the center was 4/15/19. Child #4's first day of attendance was 3/28/19.

Plan of Correction: I will get them right away. I will get the information from the parents by the end of today.

Standard #: 22VAC40-185-550-M
Description: Based on record review, the center has not maintained a complete written record of children's serious and minor injuries. Evidence: 1. 4 of 9 injury reports reviewed were incomplete. 2. On an injury report dated 4/29/19, the future action to prevent recurrence of injury was missing. 3. On an injury report dated 4/25/19, the time parents were notified was missing. 4. On an injury report dated 4/17/19, the time parents were notified was missing. 5. On an injury reported dated 4/16/19, the future action to prevent recurrence of injury and how the parent was notified was missing.

Plan of Correction: By tomorrow we will add the missing information and moving forward we will make sure all injury reports are complete.

Standard #: 22VAC40-185-570-E
Description: Based on observation, prepared infant formula bottles in the Infant Room were not properly labeled. Evidence: 1. 3 of 9 infant bottles in the Infant Room were not labeled correctly. 2. 1 infant bottle was not labeled with the child's name or dated. 3. 1 infant bottle was not dated. 4. 1 infant bottle was not labeled with the child's name. 5. 4 infants were in care at the time of inspection and are on formula.

Plan of Correction: This will be corrected right away. We will remind parents to label with names and dates.
