Comments:
An unannounced monitoring inspection was conducted today May 3, 2019 to determine the facility's compliance with licensing standards. The inspection was initiated at 9:30 am and concluded at 1:30 pm. Upon the inspector's arrival children were observed participating in International Exploration Days. Classrooms were set up to represent different countries where children engaged in a variety of activities that included: art room (France)children painted a mural, the theater (Germany) children watched a video about the accordion and danced to music, the library (Trinidad) children played the steel drums and made traditional crowns and in the gym (Mexico) children were able to look at traditional clothes, jeweler and play soccer. Children were also observed making gifts for Mother's Day, sorting dominos by the number of dots, using magnifying glasses to examine plastic bugs, working puzzles and playing with toys. Children also had time outside on the playground. A new nature outdoor play area is in use. Today's lunch consisted of jerk chicken, roasted peas, bananas, flat bread with hummus and milk. All areas of the facility were inspected to include both buildings and playgrounds. The facility has age appropriate materials and supplies. The first aid kit and non-medical emergency supplies were inspected for both buildings and all floors along with four vehicles. Staff interactions with children were positive. The following information was posted: the License, menu, daily schedule, emergency numbers and various parent information. The inspector interacted with staff and children in care. Ten children's records and seven staff records were reviewed. Medications and authorizations were reviewed. Staff have current CPR, First aid and daily health observation training. Multiple staff have MAT certification. The most recent evacuation drill was documented on 04/17/2019. The two required shelter-in-place drill for 2018 were completed on 09/17/2018 and 11/14/2018. Ratios Observed Today: (Infants) 9 infants with 3 staff. Meadow (Toddlers) 11 children with 3 staff. Forest (2 years) 14 children with 5 staff. Desert (Jr. Kindergarten) 17 children with 3 staff. Savannah (2.5-3.5 years) 17 children with 3 staff. Jungle (3.5-4.5 years) 20 children with 3 staff. Tundra (3-4 years) 19 children with 4 staff. Rainforest (5 years) 9 children with 2 staff. Description: Based on observation, the facility did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition. Evidence: Two resilient surface tiles on the toddler playground were elevated approximately one inch along one end, posing a tripping hazard.

Plan of Correction: Per the director: We are aware of the two raised tiles. Staff are also aware of the surfacing and do not allow children to play in that area. Administration has already gotten a proposal for replacement tiles and it will be fixed this summer. However, no children have been hurt on the tiles.
