A complaint was received involving supervision, ratios, physical plant, unlocked substances and preventing the spread of disease. Through observation, review of record keeping and interviews, it was determined that part of the complaint was determined to be valid. Arrival time: 2:30 pm Departure time: 5pm

Standard #: 22VAC40-185-270-A Complaint related: Yes Description: Based on observation and interview, it was determined that the inside equipment was not maintained in an operable condition. Evidence: The water fountain used by children is in disrepair as the front lever to access water is not operable. Plan of Correction: A new water fountain was ordered 4/9/19. The new water fountain will be installed no later than 4/12/19.
Standard #: 22VAC40-185-340-F Complaint related: Yes Description: Based on an interview, it was determined that individuals who are not staff members or persons allowed to pick up a child in care enter the restrooms while in use by children in the licensed program. Evidence: An interview revealed that members in the club are allowed access to the bathrooms at the same time that children in the licensed programs are in the rest rooms. Plan of Correction: Children in licensed program will only use the front rest rooms on first floor. Children in general membership program will use carpet side rest room and second floor rest rooms.
