Technical Assistance:
Technical assistance was provided in the following areas of the standards: Background checks; 22VAC40-185-(2)-70-A (Staff records); 22VAC40-185-(8)-560 (Nutrition)

Comments:
An unannounced monitoring inspection was conducted on 5/1/19 from 9:30am - 11:30am. During the inspection there were 50 children ages nine months old through three years old in care with 16 staff. Children were observed participating in various activities in the classrooms and playing outside. Records were reviewed for five children and seven staff. Medication, emergency procedures and emergency supplies were reviewed during the inspection. Areas of non-compliance are identified on the violation notice and were discussed during the exit interview.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on a review of five children's records, it was determined that the licensee did not ensure that the center shall obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: 1. The record for child #3 (22 months old) contained an immunization record that was dated 10/9/18 which was completed when the child was 15 months old which more than 6 months ago. 2. Staff #8 (Program Director) reviewed the record for child #3 and confirmed that updated immunizations were not received every six months.

Plan of Correction: The facility responded: The parents of child #3 will be contacted to provide an updated immunization record. All children under two years old will be asked to provide an updated immunization record every six months.
