Technical Assistance:
Discussion was held on the topics of current enrollment, current staffing, and documentation needed to meet a child's immunization & physical documentation needs.

Comments:
The following inspection took place between the hours of 9 am through 11:30 am. Today, I met with the Staff of the Claude Moore Recreation and Community Center located in Sterling, Virginia. At the start of this unannounced monitoring inspection there were a total of 112 children with 15 staff; good ratios in all 7 rooms. A total of 5 children?s files were reviewed. Staff files shall be reviewed at a later time through the Loudoun County PRCS main office in Leesburg. The children were engaged in the following: center activities within their classrooms, story time, art, games, field trip, large motor activities outside on playground, snack, and other organized activities. Standard #: 22VAC40-185-510-A
Description: Based on review, a medication was observed on site that did not include written parental authorization. Evidence - 1. On the date of inspection (05/03/2019), within Room #1, while the written authorization for medication to be administered to Child #1 included the doctor's written signature, it did not include the parent's written signature indicating their authorization. 2. The parent's signature line was left blank.

Plan of Correction: The authorization shall be reviewed and signed by the Parent of Child #1 at time of pick-up today.
