Comments:
An unannounced monitoring inspection was conducted from 9:00am-11:00am. Upon arrival there were 7 children in care with 2 staff present. A sample of 2 children and 2 staff records were reviewed in addition to 1 medication. Children were observed learning the alphabet and numbers with staff, playing with toys and listening to music.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a record review the center did not ensure all required information was kept for each enrolled child. Evidence: Child #1 and #2 did not have the following information on the application or records: nickname, addresses for emergency contacts and previous childcare.

Plan of Correction: The following plan of correction was provided by the owner during the exit interview: The owner will add the missing information to each child's record.

Standard #: 22VAC40-185-270-A
Description: Based on observation the center did not ensure all areas and equipment of the center, inside and outside, were maintained in a safe condition. Evidence: The wood railing at the exit of the building, leading to the playground had peeling paint and was splintered.

Plan of Correction: The following corrective action was provided by the owner during the exit interview: The owner will have the railing sanded and repainted.

Standard #: 22VAC40-185-540-C
Description: Based on observation and interview, the center did not ensure all required items were in the center's first aid kit. Evidence: The inspector asked the owner for the first aid kit the center uses during evacuation drills and in the event of an emergency. The kit the owner provided did not contain the following required items: scissors, thermometer, first aid manual or triangular bandage.

Plan of Correction: The following plan of correction was provided by the owner during the exit interview: The owner will ensure all first aid kit items are obtained and placed in the kit used for the center and in an emergency.

Standard #: 22VAC40-185-540-E
Description: Based on observation the center did not ensure all emergency supplies were available. Evidence: The center had a portable radio, however, it did not work and there were no extra batteries.

Plan of Correction: The following plan of correction was provided by the owner during the exit interview: Additional batteries will be purchased and radio will be retested to ensure it works.
