Reminder - All staff must have 16 hours of annual training per year. In addition, please fill in all blanks on children's documents. If something does not apply, write an N/A.
An unannounced monitoring inspection was conducted today from approximately 10:15am to 12:00pm. 22 children were present with 2 staff members. The physical space, required posted documents, attendance records, 2 staff records, 6 children's records, injury reports and evacuation drills were reviewed. No medications were on site. Children were observed working in small groups, listening to story time and interacting with the staff members. Interactions were positive. They also washed hands and retrieved lunch boxes from home. Lunches were properly labeled. The space was well organized with an abundant supply of age appropriate materials. Standard #: 22VAC40-185-60-A Description: Based on review of children's records, 5 out of 6 did not contain documentation of viewing proof for child's age and identity. Evidence: Copies of birth certificates were placed in all 6 children's folders, but 5 out of 6 did not have the date the original documents were reviewed by the administration or the administrator's signature confirming the document was viewed. Plan of Correction: We will ask the parents to bring in the original documents to review. We will date and sign, to provide the proof that the original was seen.
