Comments:
An unannounced monitoring inspection was conducted from 9:38am-1:00pm. At the time of entrance, 84 children were in care with 16 staff members present. Children were observed during snack time, Spanish class, Gym class, working on a Mother's day project, watching television, listening to the teacher read a book, working on a fishing pole art project and circle time. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, evacuation drills, attendance records and medications were reviewed. Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain a physical examination form for each child within 30 days of enrollment. Evidence: 1. Child #2 began at the center on 6/26/2017 and did not have the results of the child's physical.

Plan of Correction: The parent was contacted and will bring in a copy of the child's physical.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center two of eight children's records did not have complete information. Evidence: 1. Child #1 was missing the documentation of child updates and confirmation of up-to-date information in the child's record. Child #1 began at the center on 6/26/2017 and the last annual review of the child's information was dated 10/03/2017. 2. Child #2 was missing one emergency contact address and the child's physical.

Plan of Correction: Both children's parents were contacted and the information was requested.

Standard #: 22VAC40-185-270-A
Description: Based on observation, equipment of the center outside were not maintained in a safe and operable condition. Evidence: 1. The yellow and blue paint is peeling on the metal train structure outside. 2. The blue paint is peeling on the base with yellow swings. 3. The green paint is peeling on the tractor structure outside. 4. The yellow paint is peeling on the truck structure outside. 5. The swirly balance beam has a large crack/missing a part in the structure. 6. The corn hole base has peeling paint.

Plan of Correction: I will request for maintenance to sand and repaint the structures.

Standard #: 22VAC40-185-510-N
Description: Based on medication review, the center did not dispose of medication that was not picked up by the parent within 14 days after the authorization for medication expired. Evidence: 1. Child A had two medications. One medication authorization form expired on 9/15/2018 and had not been returned or disposed of. The 2nd medication did not have an authorization form. 2. Child B had a medication with a FARE form, but did not have a medication authorization form.

Plan of Correction: Both parents were contacted to update the authorization forms.
